Based on observation and interview, the licensee did not comply with the section cited above in 3 of 3 infants files reviewed were missing up to date 15 minute sleep checks which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they conduct a weeks’ worth of 15 minute sleep documentation for all infants enrolled and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025.

Based on observation, the licensee did not comply with the section cited above in that the pool gate is not working properly, specifically the self-latching mechanism on the gate is broken therefore the gate is not self-latching which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will contact pool fencing company to fix the pool gate and send proof to LPA that an appointment has been scheduled. Licensee will make the back yard off-limit until LPA verifies they are in compliance with all the pool requirements.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have documentation of conducting emergency drills with children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will conduct an emergency drill with children in care and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025.

Based on observation, the licensee did not comply with the section cited above in that the play yard that C2 was using to sleep in had a loose sheet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will purchase a fitted sheet that is the appropriate size to the mattress and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025.

Based on observation, the licensee did not comply with the section cited above in that there was a section of the mesh fencing where it connects to the concrete block wall that has a gap greater than 4 inches which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will contact pool fencing company to fix the pool fence and send proof to LPA that an appointment has been scheduled. Licensee will make the back yard off-limit until LPA verifies they are in compliance with all the pool requirements.

Based on observation, the licensee did not comply with the section cited above in that Licensee does not have a pool alarm installed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will install pool alarm and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025. Licensee will make the back yard off-limit until LPA verifies they are in compliance with all the pool requirements

Based on observation, the licensee did not comply with the section cited above in that the rescue pole is less than 12 feet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will make adjustments to ensure the pole is a fixed length of 12ft and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025. Licensee will make the back yard off-limit until LPA verifies they are in compliance with all the pool requirements.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have any documentation of the daily inspection checklist which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they conduct a weeks worth of the daily pool inspection checklist and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025.

Based on record review, the licensee did not comply with the section cited above in that Licensee's mandated reporter training expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send updated mandated reporter training certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025.

Based on record review, the licensee did not comply with the section cited above in 3 of 4 children files reviewed were missing the LIC700 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain signed LIC700s for all 3 children files and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025.

Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current roster on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee submitted updated roster while LPA was onsite; deficiency cleared onsite at the end of inspection.

Based on record review, the licensee did not comply with the section cited above in 3 of 4 children files reviewed were missing signed LIC282-Affidavit regarding Liability Insurance which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain signed LIC282s and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 06/10/2025.

Based on record review, the licensee did not comply with the section cited above in that licensee did not complete mandated reporter child care provider training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide the Department with proof child care provider training was completed, by due date 05/05/2023
