Based on observation, interview and record review, the licensee did not comply with the section cited above in that A1 is living in the home and does not have criminal background record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated A1 will complete livescan form to obtain a Criminal Record Clearance. Licensee stated they will email completed Livescan form to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/06/2026.

Based on observation, the licensee did not comply with the section cited above in that Licensee left hazardous items accessible to children in care in the backyard which poses a potential health, safety or personal rights risk to persons in care. LPA observed construction tools, machinery, and chemicals labeled "keep out of reach of children" in the backyard due to the construction of a new patio deck.

Resolution: Licensee stated they will put a temporary fencing to make the construction area inaccessible to children in care. Licensee will send photo of corrections to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/12/2026.

Based on record review, the licensee did not comply with the section cited above in that C4 did not have up to date immunization record which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send copy of C4’s up to date immunization record to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/12/2026.

Based on interview and record review, the licensee did not comply with the section cited above in that C3 did not have 15 minute sleep documentation which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send a weeks’ worth of 15 minute sleep documentation for C3 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/12/2026.
