Based on interview and record review, the licensee did not comply with the section cited above in that S2's CPR expired 09/27/2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will register S2 for CPR course and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/17/2025. Licensee will also send a certificate once course has been completed.

Based on interivew and record review, the licensee did not comply with the section cited above in that Licensee and S2 did not have complete immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit complete copy of Immunization record for themselves and S2. Licenee will send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 11/21/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that licensee and assistant do not have a current mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of mandated reporter training for licensee and assitant to the Department by due date 11/17/23.

Based on observation and interview of the licensee did not comply with the section cited above in 3 out of 8 children were infants, and assistant was left alone while Licensee left the facility for approximately 10 minutes, which poses a potential health, safety or personal rights risk to persin care.

Resolution: Licensee agreed to submit a written statement of understand of this regulation. Deficiency was correct and discussed upon Licensee's arrival.
