Based on interview and record review, the licensee did not comply with the section cited above in that Licensee does not have any documentation of emergency drills which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete an emergency drill with children in care and submit proof to LPA by 04/15/2025.
Based on interview and record review, the licensee did not comply with the section cited above in that Licensee's and Assistant's (S2) mandated reporter training expired June 2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit current mandated training certificate for themselves and Assistant. Licensee will submit to LPA by 04/15/2025.
Based on interview and record review, the licensee did not comply with the section cited above in that assistant's (S2) CPR expired July 2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will register their Assistant for CPR class by 04/11/2025 and submit proof to LPA . Licensee will submit certificate once the course is completed.
Based on interview and record review, the licensee did not comply with the section cited that Licensee did not have complete 15 minute sleep check for infant (C4) which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee last documented sleep check for the infant in November of 2024, C4 will turn 24 months November 2025.
Resolution: Licensee will submit a weeks worth of 15 minute sleep check documentation for infant in care. LPA will submit to LPA by 04/15/2025.
