Based on record review, the licensee did not comply with the section cited above in that S2 had documentation of Basic CPR on file which poses a potential health, safety or personal rights risk to persons in care. S2 assists with transporting children.

Resolution: Licensee stated they will register S2 for correct CPR course that includes Pediatric CPR/First Aid and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/21/2026. Licensee will also send a certificate once course has been completed.

Based on interview and record review, the licensee did not comply with the section cited above in that licensee does not have a current mandated reporter training on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide proof of completed mandated reporter training to the Department by due date 02/05/2024.

Based on observation and interview, the licensee did not comply with the section cited above in that a wooden play structure is not in good repair which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will remove the play structure from the oudoor play area and will send proof to the Department by due date 02/22/2024.
