Based on record review, the licensee did not comply with the section cited above in that Licensee's Mandated Reporter expired 01/17/2026 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send updated mandated reporter training certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/28/2026.

Based on record review and interview, the licensee did not comply with the section cited above in that C2 and C5 did not have LIC9227 form (Individual Infant Sleeping Plan) on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain signed LIC9227 for C2 and C5. Licensee will send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/28/2026.

Based on interivew and record review, the licensee did not comply with the section cited above in that Licensee's CPR expired 11/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will enroll for CPR class by 1/15/2024 and send certificate of completion to LPA by 01/31/2025.

Based on record reveiw and interview, the licensee did not comply with the section cited above in that 2 of 3 staff members mandated reporter training expired 01/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit updated mandated reporter training certificates for both staff members, S1 and S3.

Based on observation and record review, the licensee did not comply with the section cited above in that License was alone with 8 children (2 infants/6 toddlers) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will provide a written statement attesting she understands must comply with the capacity of a small family child care home when there is no assistant present at her home which is licensed as a large.

Based on interivew and record review, the licensee did not comply with the section cited above in that S3 did not have doumentation of any of the required immunization on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit S3's immunization record that will include all required immunization.

Based on interview and record review, the licensee did not comply with the section cited above in that S3 did not have documentation of criminal clearance which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit documentation of clearance to LPA via email by 1/15/2025.
