Based on interview and record review, the licensee did not comply with the section cited above in S2 has eligible fingerprint clearance however was not associated to new facility license number which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will associate S2 to Licensee’s new facility number and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/04/2026. If Licensee cannot associate S2, Licensee will have S2 redo fingerprints.
Based on record review, the licensee did not comply with the section cited above in that C1 and C5 did not have immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send copy of C1 and C5 complete immunization record to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/10/2026.
