Based on observation, the licensee did not comply with the section cited above in that infant (C04) was sleeping with a blanket in the play pen which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will review infant safe sleep regulations and submit statement to LPA confirming they have read and understand infant safe sleep regulations. Licensee will submit to LPA by 02/19/2025.
Based on observation,interview,record review, the licensee did not comply with the section cited above in that adult living in the home (A1) was observed supervising children and did not have mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states will submit proof of mandated reporter training completed by A1, to the Department by due date on 03/08/2024.
