GODINEZ-RAMOS FAMILY CHILD CARE

Based on interview and record review, the licensee did not comply with the section cited above in that S2's CPR expired 03/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will register S2 for CPR class and send proof to LPA by 03/28/2025. Licensee will send certificate to Licensing once class is completed. Licensee stated they will not leave S2 alone with any children until class has been completed.

Based on interview and record review, the licensee did not comply with the section cited above in that S2 transports children from school to the Family Child Care Home and does not have a valid drivers license which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated S2 will no longer transport children until they have a valid drivers license. Licensee stated they will transport children if needed. Licensee will submit statement to LPA with plan of operation.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that Licensee has not completed the General training and has not renewed the Child Care training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the General and Child Care Mandated Reporter Training and submit proof of completion to the Department by 5/25/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that Licensee is not conducting drills with the children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conducted a fire/disaster drills with the children document and submit proof of completion to the Department by 5/25/2023.
