Based on record review and interview, the licensee did not comply with the section cited above in that Licensee was not documenting the time of each 15 minute check for infants in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send a weeks’ worth of complete 15 minute sleep documentation for all infants in care to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 03/10/2026.
Based on record review, the licensee did not comply with the section cited above in that C1 and C2 did not have signed LIC9224 on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will submit proof of completed LIC9224s for all newly children enrolled. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 03/10/2026.
Based on observation, the licensee did not comply with the section cited above in that an outlet cover in play area is missing the cover and in not in good repair which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states will replace the outlet cover and submit proof to the Department by 02/09/2024
Based on record review, the licensee did not comply with the section cited above in 3 out of 11 children were not listed on the current Roster, lic9040 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee update the Roster during the visit and added the 3 missing children. Correction has been completed on this date.
