Based on interview and record review, the licensee did not comply with the section cited above in that Licensee's mandated reporter training expired 11/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send updated mandated reporter traianing certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 04/17/2025.
Based on interview and record review, the licensee did not comply with the section cited above in that 1 of 1 infant enrolleed did not have 15 minute sleep log documentation on file which poses potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send a weeks worth of sleep log documentation for infant in care. Licensee agrees to send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 04/17/2025.
Based on interview and record review, the licensee did not comply with the section cited above in that S1 is living in home did not have TB test on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send proof of S1's TB clearance. Licensee agrees to send proof of S1's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 04/17/2025.
Based on observation and interview, the licensee did not comply with the section cited above in that one infant (C4) was observed with loose objects in the play yard/sleeping area, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee removed all loose objects from the play yard at the time of the inspection.

Resolution: Licensee agrees to have assistant complete mandated reported training to the Department by due date 04/28/2023.
