Based on record review, the licensee did not comply with the section cited above in that C3 did not have LIC627 form (Consent for Emergency Medical Treatment) on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send signed LIC627 form (Consent for Emergency Medical Treatment) to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/24/2025.

Based on observation, the licensee did not comply with the section cited above in infant in care was sleeping with blankets in the play yard which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will review safe sleep regulations and send email to LPA confirming they have read and understand all safe sleep regulations. Licensee immediately removed blankets from play pen while LPA was onsite.

Based on record review and interivew, the licensee did not comply with the section cited above in that the last documented emergency drill was completed on 05/2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will conduct an emergency drill with children in care and submit proof of documentation to LPA by 10/16/2024.

Based on record review and interview, the licensee did not comply with the section cited above in that Licensees mandated reporter training expired 08/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will complete mandated reporter training and submit certificate of completion to LPA by 10/16/2024.

Based on observation and record review, the licensee did not comply with the section cited above in that Licensee did not have updated roster which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will update the roster and send copy to LPA by 10/16/2024.

Based on interview and record review, the licensee did not comply with the section cited above in that the last documented sleep log for the infant in care was 08/01/2024 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send LPA a weeks worth of sleep log documentation for the infant in care.

Based on observation and interview, the licensee did not comply with the section cited above which poses, due to a carbon monoxide detector being observed in the home, however it was not installed and had no batteries. In addition, there were two smoke detectors observed and one was not in working order and appeared to need battery replacement , which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to bring the carbon monoxide detector and smoke detector to working order by due date 08/16/2022 and will send proof of correction to LPA Valenzuela.

Based on records review and interview, the licensee did not comply with the section cited above, due to the licensee states she has not completed the mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide proof of mandated reporter training to LPA by due date 08/16/2022.

Based on observation and interview, the licensee did not comply with the section cited above, due to there being 10 children in care at the time of the inspection, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During time of inspection, licensee called two parents to pick up two day care children to ensure was within capacity. Licensee will ensure that she does not exceed the licensed capacity in the future.

Based on observation, the licensee did not comply with the section cited above in due to cleaning chemicals were observed accessible in the bathroom cabinet and knives were observed accessible in a kitchen drawer, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the cleaning chemicals from the bathroom and made the knives inaccessible at the time of the inspection.
