Based on observation and interview, the licensee did not comply with the section cited above in that play structure is not in good repair in several areas, metal flooring was observed plastic coating to be missing, with areas having raised pointed parts. Also at least two nuts or bolts were missing. In addition a slide surface is broken, which poses a potential health, safety or personal rights risk to persons in care.
