Based on record reivew, the licensee did not comply with the section cited above in that Licensee was not able to provide complete documentation of 15 minute sleep logs for C3 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will review infant safe sleep documentation and send statement confirming they have read and understand infant safe sleep regulations. Licensee was previously cited on 02/20/2025 this is a repeat deficiency and a civil penalty has been assessed for $250.00.
Based on record review, the licensee did not comply with the section cited above in that C4 did not have LIC282 signed which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send signed LIC282 form (Affidavit Regarding Liability Insurance) for C4 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/25/2026.

Based on observation, the licensee did not comply with the section cited above in that C1 was sleeping in a baby swing set which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed infant from baby swing set once LPA advised them of the regulation. Licensee will review and read infant safe sleep regulations and send statement of understanding to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/25/2026.
Based on observation, the licensee did not comply with the section cited above in that Licensee planted new plants in the backyard with sharp thorns (orange/Lime trees) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will make the plants inaccessible and send photos to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/25/2026.
Based on observation, the licensee did not comply with the section cited above in that the play equipment in the backyard had broken pieces which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will replace replace broken pieces on the play ground equipment and submit photo update to LPA by 02/27/2025. Licensee stated they will make off-limti until it is safe for children in care.

Based on observation, the licensee did not comply with the section cited above in that Licensee has a cold plunge spa that had approx. 1 ft water. The spa had a cover with no locks which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee drained the spa while LPA was onsite. Licensee will install cover that has locks to ensure it is inaccessible to children in care. Licensee will send photos to LPA by end of day, 02/21/2025.

Resolution: Licensee removed all the bottles during inspection and made them inaccessible during visit.
