Resolution: Licensee will review infant safe sleep documentation and send statement confirming they have read and understand infant safe sleep regulations. Licensee was previously cited on 02/20/2025 this is a repeat deficiency and a civil penalty has been assessed for $250.00.

Resolution: Licensee drained the spa while LPA was onsite. Licensee will install cover that has locks to ensure it is inaccessible to children in care. Licensee will send photos to LPA by end of day, 02/21/2025.
