Based on record reivew, the licensee did not comply with the section cited above in that Licensee was not able to provide complete documentation of 15 minute sleep logs for C3 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will review infant safe sleep documentation and send statement confirming they have read and understand infant safe sleep regulations. Licensee was previously cited on 02/20/2025 this is a repeat deficiency and a civil penalty has been assessed for $250.00.

Based on record review, the licensee did not comply with the section cited above in that C4 did not have LIC282 signed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send signed LIC282 form (Affidavit Regarding Liability Insurance) for C4 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/25/2026.

Based on record review, the licensee did not comply with the section cited above in that C2 did not have up to date immunization record which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send confirmation of C2's next doctor appointment to ensure they are on track to receive their most up to date required immunization. Licensee will send to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/25/2026.

Based on observation, the licensee did not comply with the section cited above in that C1 was sleeping in a baby swing set which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed infant from baby swing set once LPA advised them of the regulation. Licensee will review and read infant safe sleep regulations and send statement of understanding to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/25/2026.

Based on observation, the licensee did not comply with the section cited above in that Licensee planted new plants in the backyard with sharp thorns (orange/Lime trees) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will make the plants inaccessible and send photos to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 02/25/2026.

Based on observation, the licensee did not comply with the section cited above in that the play equipment in the backyard had broken pieces which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will replace replace broken pieces on the play ground equipment and submit photo update to LPA by 02/27/2025. Licensee stated they will make off-limti until it is safe for children in care.

Based on record review and interview, the licensee did not comply with the section cited above in that Licensee's CPR expired 02/07/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will register for CPR course by 02/22/2025 and send updated certificate to LPA once the course has been completed.

Based on record review, the licensee did not comply with the section cited above in that 5 of 6 children files reviewed were missing immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit immunization record for the 5 of 6 children files that were reviewed today. Licensee will submit to LPA by 02/27/2025.

Based on record review, the licensee did not comply with the section cited above in that 2 of 2 infant files (C3 and C6) reviewed were missing 15 minute sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit a weeks worth of complete 15 minute sleep logs for C3 and C6 to LPA by 02/27/2025.

Based on observation, the licensee did not comply with the section cited above in that Licensee has a cold plunge spa that had approx. 1 ft water. The spa had a cover with no locks which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee drained the spa while LPA was onsite. Licensee will install cover that has locks to ensure it is inaccessible to children in care. Licensee will send photos to LPA by end of day, 02/21/2025.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 records, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of immunization records for S1 and S2 to LPA by 3/24/2023

Based on record review, the licensee did not comply with the section cited above in Child #1 did not have the required Individual Infant Sleeping Plan, LIC9227, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain Individual Infant Sleeping Plan, LIC9227 for Child #1 and submit a copy to the Department by 3/8/2022.

Based on record review, the licensee did not comply with the section cited above in the current roster, lic9040 had several children missing dates of birth, last names and none of the children who no longer attend the facility had the dates left, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update the Roster, LIC9040 with all of the missing information and submit a copy to the Department by 3/8/2022.

Based on observation, the licensee did not comply with the section cited above in that in the children restroom under the sink was a total of 5 bottles of Purell Surface Sanitzer, which contained warning label, keep out of reach of children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed all the bottles during inspection and made them inaccessible during visit.
