Based on record review, the licensee did not comply with the section cited above in that S2 did not have MMR or Influenza on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send copy of S2's completed immunization record to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/14/2025.
Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current roster on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and observation, the licensee did not comply with the section cited above in that Licensee was left alone with 8 children (1 infant; 7 toddlers) for approximately 10 minutes which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send a statement of understanding of staffing and ratio regulations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/14/2025.
Based on interview and record review, the licensee did not comply with the section cited above in that Licensee's CPR expired 09/09/2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will register for CPR course and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/14/2025. Licensee will also send a certificate once course has been completed.
Based on record review and interview, the licensee did not comply with the section cited above in that Licensee and S2's mandated reporter training expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send updated mandated reporter training certificates to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/14/2025.
Based on observation,interview and record review, the licensee did not comply with the section cited above in that there was seven children in care, with all children being four years or younger and with two children being infants, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: At time of inspection, one child (Child #6) was picked up by parent.

Resolution: Licensee agrees to complete LIC 700 and LIC 627 for Child #6, and Child #7, and will submit copies to the Department on 10/11/2022.
