Based on interview and record review, the licensee did not comply with the section cited above in that A1, A2 and A3 are living in the home and did not have tuberculosis clearance on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to send proof of A1, A2, A3 TB's clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/30/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that C3 did not have LIC9227 on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will obtain signed LIC9227 and send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/23/2025.

Based on record review and interview, the licensee did not comply with the section cited above in that C2, C3 and C5 did not have complete 15 minute sleep logs on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send two weeks’ worth of 15 minute sleep documentation of all infants in care to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 12/30/2025.

Based on interview and record review, the licensee did not comply with the section cited above in licensee and assistant does not have a mandated reporter training on file at time of inspection this poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will take mandated reporter training and submit proof of completed training to the Department by due date 12/22/2023.
