BARAJAS FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in that infant in care (C2) was in the car seat asleep which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed C2 from the car seat after LPA informed them of the regulation. Both Licensee's will review infant safe sleep regulations and submit email confirming they have read and understand all safe sleep regulations. Licensees will submit by the end of the business day on the Plan of Correction (POC) due date of 05/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above in that both Licensees’ mandated reporter training expired 09/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send updated mandated reporter training certificates to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 05/01/2025.

Based on record review and interview, the licensee did not comply with the section cited above in that Licensee did not have any 15 minute sleep log documentation for C2 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send a weeks’ worth of 15 minute sleep documentation of all infants in care to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 05/01/2025.

Based on interview and record review, the licensee did not comply with the section cited above, due to both licensees not having completed mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Biviana Barajas agress to provide proof of mandated reported training for both licensees and submit certificates of general and child care training to the Department by due date 09/20/2022.
