Based on record review, the licensee did not comply with the section cited above in that S2 did not have fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee contacted another assistant to provide assistance with coverage. S2 left the FCCH to obtain fingerpint clearance. Licensee will send proof of Livescan form to CCLD by end of day on 07/16/2025. Licensee understands S2 cannot work until S2 receives fingerprint clearance.
Based on record review, the licensee did not comply with the section cited above in that staff did not have complete immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will send proof of complete immunization record for staff to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/23/2025.
