MONTIEL, NICOLE FAMILY CHILD CARE

Based on records review, LPA observed that 2 out of 3 staff AB1207 trainings were expired, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure that all staff take the AB1207 training by POC deadline and submit a copy of proof to CCLD Fresno.

Based on file reviews LPA Herrera observed that licensee and staff #2 had expired their CPR/First Aid certifictaion expired. Staff #3 was present with a current CPR/First Aid, however she left the facility during the inspection.

Resolution: Licensee Montiel and staff #2 will register to take the CPR/Fist Aid training and provide proof of registration to LPA Herrera along with the updated CPR/First Aid card once the training is completed.

Based on observation, the licensee did not comply with the section cited above in which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will read and review capacity and ratio regulations for a large family child care home. Licnesee to write a statement regarding capacity date, sign and submit to LPA Gibson by plan of correction date 4/26/23

Based on record review the licensee did not comply with the section cited above for two chidlren's files out of 6. Licensee did not have a file for child 6, child 5 was missing emergency consent signature from authorized representative which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get a complete file for the child 6 complete with immunizations, LIC 627, LIC700 and a signed LIC627 for Child 5.

Based on record review the licensee did not comply with the section cited above in Licensee did not a signed consent of LIC 995E in file for child 6. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get a complete file for the child 6 to include form LIC 995E.
