Based on LPA observations, the licensee did not comply with the section cited above as 2 infants were sleeping in separate rooms with the door closed which prevent the provider to visually observe the children sleeping, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have the door open at all times while infants sleep. Licensee agrees to submit a written statement to LPA stating that she understands the door to the room the infants sleep in shall remain open at all times. The written statement will be submitted by email/text by 01/12/2026.

Resolution: Licensee Bravo stated that she will ensure to submit the Mandated Reporter Training certificates for all staff to CCLD Fresno by 01/02/2025.

Based on record review, LPA Herrera observed that the licensee did not comply with the section cited above as all three infants enrolled did not have a 15-minute sleeping log on file that documeneted 15 minute checks, with date, time and date, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Bravo stated that she will ensure to document her 15 minute sleep checks in logs. Licensee stated that she lost her file but will ensure to provide 15 minute sleeping logs for all three infants and submit proof to CCLD Fresno by 01/02/2025.

Resolution: Licensee stated she will have Staff #1 obtain documentation of a current health screening and provide documentation to Community Care Licensing by 03/15/2024.

Resolution: Licensee removed the items during today's inspection and made them inaccessible to children. Licensee stated she shall submit a written stated confirming understanding of infant items that are not allowed in family child care homes by POC date 04/08/2022.
