Type B citation - potential risk if not corrected

Based on staff interviews menus were not posted prior to April 2020. This poses a potential health and safety risk to the children in care.

Based on staff interviews staff corroborate that on more than one occasion the nap mats in the toddler room did not have enough space to walk between them and were sometimes touching. This poses a potential health and safety risk to the children in care.

Based on staff interviews the preschool has had more than 15 children, and up to 20 children in care on more than one occasion with one qualified teacher and 1 or two aides that do not hold units in Early Childhood Education. This poses a potential health and safety risk to the children in care.

Based on interviews conducted and records reviewed S1 has yelled at day care child(ren) in care on more than one occasion, which poses a potential personal rights risk to the children in care.

Based on staff interviews rodent droppings and spiders have been observed in the facility in March 2020. Based on record review the facility is serviced for ants/carpenter bees only. This poses a potential health and safety risk to the children in care.

Based on staff interviews D1 pull on the arms of multiple children on more than one occasion, handling the children in care roughly. This poses an immediate health and safety risk to the children in care.

Based on interviews conducted S1 has handled day care child in care in a rough manner on more than one occasion, which poses an immediate health, safety and personal rights risk to the children in care.

Based on staff interviews and record review A1 and S6 were working and not associated to the facility and a transfer request was not completed which poses a potential safety risk to the children in care.

Based on staff interviews, on more than one occasion rusty nails and screws were found by children on the playground after it was brought to L1’s attention. This poses an immediate health and safety risk to the children in care.

Based on interviews with L1 and S1-S3 and S5-S6, the hot water heater in the school age room has an issue that causes the sink water to smell like rotten eggs, and the preschool children use the school age bathroom sink when playing outside to wash their hands. This poses a potential health and safety risk to the children in care.

Based on staff interview, L1 has instructed staff that they may not communicate with children’s authorized representative’s, injuries have occurred, and L1 was not present at the facility to report the incident or chose not to report an incident to a child’s authorized representative. This poses a potential health and safety risk to the children in care.

Based on staff interviews milk is only served to the children if they request it and a milk alternative is not served to children with milk allergies.This does not meet the requirement for each meal to include milk. This poses a potential health and safety risk to the children in care.

Based on interview and record review, the licensee did not comply with the section cited above and in the last 30 days has not hired a qualified Director after the former Directors last day on 07/19/22 which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above in two out of three staff files. Staff L1 and S2 did not have current Mandated Reporter Training certificates on file which poses a potential health, safety or personal rights risk to persons in care.

Based on interview with L1 at 10:18am and record review of staff files the facility has a change of Director, L1 stated that she is now qualified to be the Director and has not yet notified the Department, which poses a potential health, safety or personal rights risk to persons in care.
