Resolution: During the visit Licensee's daughter purchased a new fitted sheet and it was replaced.

Resolution: Licensee will have the parent fill out the form and provide a copy to the LPA.

Resolution: Licensee and Licensee's Assistant will take the mandated reporter training and send a copy of the completed certificates via email to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above. LPA observed soap, an electric shaving razor, and caulk in the drawers below the sink of the bathroom the children use which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed the soap, electric shaving razor, and caulk and placed them in an inaccessible cabinet.
