Based on observation and intervieww hich poses an immediate health, safety or personal rights risk to persons in care. LPA observed an adult assistant without a criminal record clearance

Resolution: Adult assistant left the childcare facility, when another assistant showed up, who had a cleared criminal history and association to the facility.

Based on record review, the licensee did not comply with the section cited above in that the assistant did not have mandated reporter training completed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to email LPA L. Habtom a copy of the mandated reporter training for her assistat POC date.

Based on observation, the licensee did not comply with the section cited above in that there were 5 infants and 14 children with no school age which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee was advised of ratio guidelines and one infant was picked up while LPA was present. Licensee made parents aware that she can no longer care for that infant until one child turns two.

Based on record review, the licensee did not comply with the section cited above in that licensee is not documentating the nap checks which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to email LPA L. Habtom copies of the nap checks by POC date.

Based on observation, the licensee did not comply with the section cited above in that a boppy sleeper was being used for an awake infant who ended up falling asleep which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the boppy sleeper and was made aware to not use any recalled items in the day care. Licensee was advised to check for recalled items.

Based on observation, the licensee did not comply with the section cited above in that two infants were in pack n plays with blankets which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the blankets and was made aware that infants in pack and plays may not have blankets.

Based on observation, the licensee did not comply with the section cited above in that an infant was sleeping in a car seat with a blanket on top which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee was made aware that infants cannot sleep in car seats and must be moved into an approved sleep environment once asleep.

Based on observation the licensee did not comply with the section cited above in when LPA observed a 10 month old sleeping in a play yard which contained loose objects (a full bottle and two small soft toys) which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: licensee immediately removed the objects from the play yard during today's inspection.

Based on observation, the licensee did not comply with the section cited above in when LPA observed a 10 month old infant asleep in a playpen with the a sheet that was too big for the mattress resulting in the sheet was bunched up in many areas around the sleeping infant which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee was advised of the danger of the loose sheet bunching up and blocking the infants mouth/nose. When LPA pulled out the sheet it appeared to belong to a twin size bed. Licensee states she will purchase a sheet designed for a playpen or crib that is the right size for the sleeping area. To correct the deficiency, licensee immediately removed the sheet and states she will purchase an appropriate fitting sheet for the play yard. To correct the deficiency, she shall submit a picture of the new fitted sheet to LPA within 24 hours.
