Teacher-Ratio: The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance. This requirement has not been met by evidence: LPA observed the facility with 21 preschool children with the director and aide making the facility out of ratio on 3/29/21. This is considered as an immediate risk to the children in care.
Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met, as evidenced by: Based on interviews and observations, the building was not in good repair with paint cipping observed on doorframes, windowills, and boarders. This poses a potential risk to the health and safety of children in care.

This poses an immediate risk to the health and safety of children in care.
Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 5 staff. Director stated Staff memebr started in December of 2023 which poses a potential health, safety or personal rights risk to persons in care.

Director stated hire date was in December of 2023 which poses a potential health, safety or personal rights risk to persons in care.
(e) Each infant's bedding shall be used for him/her only. Such bedding shall be replaced when wet or soiled, or when the crib, mat or cot is to be occupied by another infant. (1) Bedding shall be changed daily, or more often if required by subdivision (e) above.
(g) A photocopy of the teacher's Child Development Permit as specified in (c)(3) above, or a photocopy of the teacher's transcript(s) documenting successful completion of required course work, shall be maintained at the center.

This requirement was not met, as evidenced by: Based on interviews and observations, the child care center director or substitute director was not always on the premises during center operating hours. This poses a potential risk to the health and safety of children in care.
