(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

Resolution: Assistant will provide proof of mandated training to LPA via e-mail.

Based on observation, the licensee did not comply with the section cited above in that bug spray killer was in the backyard with the gate opened on the ground which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the bug spray killer from the backyard and placed in the locked garage. Licensee was reminded to lock all poisonous items locked away.

Based on record review, the licensee did not comply with the section cited above in that last fire drill documented was in December 2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to conduct and document a fire drill. Licensee agreed to email a copy of the fire drill by POC date of 2/24/2023.

Based on record review, the licensee did not comply with the section cited above in that the two assistances did not have mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have two assistances complete mandated reporter training and email to LPA by POC date of 2/24/2023.

Based on record review, the licensee did not comply with the section cited above in that the two assistances that were left alone with the children prior to licensee's arrival did not have proof of CPR which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee got to the home wihtin 10 minutes of the inspection and was made aware to not leave assistances alone that do not have CPR ceritification.

Based on record review, the licensee did not comply with the section cited above in that both assistances did not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide copies of immunizations of assistances to LPA by email by POC date of 2/24/2023.

Based on record review, the licensee did not comply with the section cited above in that licensee is not documentating infant 15 minute sleep checks which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to email LPA copies of 15 minute sleep checks by POC date of 2/24/2023.
