Based on record review, the licensee did not comply with the section cited above in there is no documentation for 15 minute sleep checks, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return to observe documetation of 15 minute sleep checks.

Based on observation and interview, the licensee did not comply with the section cited above in four children stated they were in the garage, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA will return to observe off-limit areas are not being used for child care.

Based on observation, the licensee did not comply with the section cited above in hazardous items were found accessible to children including nails, unfenced tools, wired fence, lawn mower, and broken plastic poles, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove all hazardous items accessible to children. LPA will return to complete observation.

Based on observation, the licensee did not comply with the section cited above in 16 children were observed in care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA will return to ensure proper capacity is being followed.

Based on record review, the licensee did not comply with the section cited above in there is no record of a fire drill being completed within the last six months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return to observe record of completed fire drill.

Based on record review, the licensee did not have a file for the assistant, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return to observe completed file for assistant.

Based on record review, the licensee did not comply with the section cited above in licensee does not have record of emergency information for multiple files or consent to emergency medical care for 4 of the 5 files reviewed, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will return to observe completed childrens files.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as documentation of napping checks has not been conducted, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide documentation of napping logs by POC due date.
