Based on record review, the licensee did not comply with the section cited above in that two children did not have record of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email proof of immunizations to LPA L. Habtom by POC due date.

Based on record review, the licensee did not comply with the section cited above in that CPR certificate was not renewed and expired on 10/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email the copy of the updated CPR ceriticate to LPA L. Habtom by POC due date.

Based on observation, the licensee did not comply with the section cited above in that hazadorus items such as knifes, hair products and body products had baby child locks that were not operable making items accessible to children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed all items from the bathroom drawer and placed new child locks in the kitchen cabinet where knifes are stored,

Based on observation, the licensee did not comply with the section cited above in that a prohibited item, baby bouncer, was in the daycare area which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the baby bouncer from the daycare area. LPA L. Habtom will make a return visit to confirm item is not in daycare area.

Based on observation, the licensee did not comply with the section cited above in that 3 infants had blankets in the pack n play during nap time which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed blankets from pack n plays and agreed to not provide infants in pack n plays with blankets. LPA L. Habtom provided infant safe sleep regulations. LPA L. Habtom will make a return visit.

Based on observation, the licensee did not comply with the section cited above in that 1 infant was laying asleep with no indication of ability to roll on LIC 9227 from stomach to back and back to stomach in the pack n play which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee was made aware to place infants on their backs to sleep until they are able to roll from back to stomach and stomach to back and to complete LIC 9227.

Based on record review, the licensee did not comply with the section cited above in that sleep logs were not documentated for all attending infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to email copies of sleep logs to LPA L. Habtom via email by POC date.

Based on record review, the licensee and her assistant did not comply with the section cited above with providing proof of mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete the training and provide proof of mandated reporter training to LPA L. Habtom via email.
