Based on observation, interview and record review, the licensee did not comply with the section cited above in licensee was alone caring for eight (8) children; none of whom were attending kindergarten or 6 years of age which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will ensure there is an assistant at the facility in order to maintain capacity requirements. Licensee stated she will utilize her available substitute assistant if needed.
Based on interview and record review, the licensee did not comply with the section cited above in the licensee and helper did not have a current mandated reporter training certificate on file for review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit a current mandated reporter certificate for her and her helper to the SDRO by 5/18/23. Licensee stated she will make note on her calendar to ensure the certificate is renewed every two years.
Based on interview and record review, the licensee did not comply with the section cited above in one (1) out of one (1) helpers did not have complete immunization records on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit proof of pertussis immunization records for her helper to the SDRO by 5/18/23.
Based on interview and record review, the licensee did not comply with the section cited above in licensee did not have a sleep log for one (1) out of one (1) sleeping infants which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will begin logging 15 checks as of today and send proof to the SDRO of one (1) weeks checks to the SDRO by 5/18/23.
