Based on LPA observation and Licensee interview, the licensee did not comply with the section cited above as an infant was sleeping inside a crib that contained blankets and stuffed animals, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During inspection licensee removed items from the crib. LPA reminded Licensee of Safe Sleep Regulations and provided PIN 20-24 CCP. Licensee stated that she understood.

Based on LPA observations and Licensee interview, the licensee did not comply with the section cited above as an infant was sleeping inside a crib which had blankets hanging on the side, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During inspection licensee removed items hanging from the crib. LPA reminded Licensee of Safe Sleep Regulations and provided PIN 20-24 CCP. Licensee stated that she understood.

Based on LPA obervations, the licensee did not comply with the section cited above as a teeter totter in the back play yard was situated on top of concrete without protective padding underneath the fall zones, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that place protective rubber padding with an artificial grass cover will be placed under fall zones. Licensee will provide LPA with photos of proof of correction via email or text by 04/26/2022.

Resolution: During inspection Licensee made hazardous items inaccessible to children in care.

Based on LPA observations, the licensee did not comply with the section cited above as a can of Raid insecticide was not made inaccessible in the back play yard. No children were present in the back play yard during the inspection. This poses a potential health and safety risk to persons in care.

Resolution: During inspection Licensee secured poison in a locked cabinet located in the garage.
