Based on interview and record review, the licensee did not comply with the section cited above in that the child roster on file is not up to date, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will update their child roster and submit proof to the SDRO by 12/19/2025.

Based on observation, the licensee did not comply with the section cited above in that they were absent from the facility while their 15yr old assistant was present with a daycare child, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will draft a plan which outlines child drop off and morning pick up schedules to ensure children are not dropped off in their absence, and submit a copy of the plan to the San Diego Regional Office (SDRO) by 12/19/2025.

Based on observation, the licensee did not comply with the section cited above in that they did not have a regulation complaint fire extinguisher readily available, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will obtain a fire extinguisher rated 2A:10B:C or higher and submit proof of installment to the SDRO by 12/19/2025.

Based on record review and interview, the licensee did not comply with the section cited above in that the last fire drill was recorded on 06/19/2024, which is longer than 6 months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will conduct a fire drill with daycare children and submit proof to the San Diego Regional Care Office, (SDRO) by 12/19/2025.

Based on observation and record review, the licensee did not comply with the section cited above in that the licensee did not have a current mandated reporter certificate available for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will take the mandated reporter training and submit a copy of the certificate to the San Diego Regional Office (SDRO) by 12/19/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that they did not have a current CPR certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will register for CPR training and and submit registration to the SDRO by 12/19/2025.

Based on observation and interview, the licensee did not comply with the section cited above in that they have commenced an alteration to the family child care home without reporting to the department, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will submit an updated LIC999A along with a LIC279 listing all adults currently living in the home to the SDRO by 12/19/2025.

Based on observation and record review, the licensee did not comply with the section cited above in that two (2) out of two (2) children in care did not have an updated PM 286 record on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they would review the provided LIC311D and obtain missing records and submit proof of complete child files to the SDRO by 12/19/2025.

Based on interview record review, the licensee did not comply with the section cited above in one out of one person which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Assistant Karina Santiago will submit prints to LIVE SCAN office and will be associated to licensee's cleared list prior to assisting at the facility.
