PACHECO, YOVANA FAMILY CHILD CARE

Based on LPA observation, the licensee did not comply with the section cited above in where the LPA observed an area under the slide on the outdoor playset that has been dug out that needs to be made inaccessible to children in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will make the area inaccessible to children in care and will provide proof the LPA by 08/14/2025.

Based on licensee interview and record review, the licensee did not comply with the section cited above in four (4) out of four (4) infants sleep logs, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will submit proof of sleep logs for the four (4) infants by the end of the day on 07/16/2025. The licensee also stated she will provide the LPA proof of one month of sleep logs by 08/14/2025.

Based on LPA observation and licensee interview, the licensee did not comply with the section cited above in one (1) out of one (1) play yard which had a loose fitting sheet which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will return the sheet to the parents and will provide the LPA proof of the play yard with a fitted sheet or no sheet at all by 08/14/2025.

Based on LPA observation, the licensee did not comply with the section cited above in one (1) out of one (1) play yard and one (1) out of one (1) bassinet which had items such as blankets, infant feeding boppy and a toy, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated the items are not normally inside the play yard or bassinet but will refrain from placing any items inside when the infants are napping. The licensee will submit proof of cleared play yard and bassinet to the LPA by 08/14/2025.

Based on LPA observation, the licensee did not comply with the section cited above in which the LPA observed a Baby Einstein Ocean Explorers jumper in the living room, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will dispose of the jumper and will submit proof the item is gone by 07/16/2025 to the LPA.

Based on interview and record review, the licensee did not comply with the section cited above due to staff present in the facility has not taken the required AB-1207 Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant Stephanie stated that she will ensure that she and the other staff takes the required Mandated Reporter training and will ensure to send a copy of the certificate to the SDCCRO by the due date of 10/20/2023.

Based on interview and record review, the licensee did not comply with the section cited above by not having the required Individual Infant Sleeping Plan on infant child under 12 months present at the facility, which poses potential health, safety or personal rights risk to persons in care.

Resolution: Assistant Stephanie Lopez, stated that she will ensure to provide the LIC-9227 to parents and together with them fill out the plan. Stephanie stated she will send a copy of the completed form to the San Diego Child Care Regional Office (SDCCRO) by the due date as proof of correction.

Based on interview and record review, the licensee did not comply with the section cited above due to staff not knowing that they have to document naps for children under 24 months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant Stephanie Lopez, stated that she will ensure to start documenting the nap time and note the 15 mins check on all infants under 24 months. Assistant will send a copy of the Napping log to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.
