Based on observation, interview and record review, the licensee did not comply with the section cited above in that one (1) out of four (1) staff did not have a fingerprint clearance and have been working at the facility for four (4) to six (6) months, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will send staff #1 (S1) home and not allow them to return until they obtain a fingerprint clearance. Licensee stated that they will send proof of S1's fingerprint clearance to the San Diego Regional Office (SDRO) by 05/16/2025.
Based on observation, the licensee did not comply with the section cited above in that knives were found in a drawer located in the kitchen that is accessible to children, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will move knives to a higher location, making the knives innaccessible to children in care. Licensee stated that they will provide proof of secured knives to the San Diego Regional Office (SDRO) by 05/16/2025.
Based on observation, the licensee did not comply with the section cited above in that cleaning chemicals were found under the kitchen sink that were accessible to children in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will move chemicals to a different location, making the chemicals innaccessible to children in care. Licensee stated that they will provide proof of secured chemicals to the San Diego Regional Office (SDRO) by 05/16/2025.
