Based on observation and interview, the licensee did not comply with the section cited above in that the door leading to the garage, located in the living room, is unsecured making the off limits area accessible, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they would secure the door leading to the garage with a door handle cover, making the off limits area innaccessible to children in care. Licensee stated that they would submit proof to the San Diego Regional Office (SDRO) by 04/18/2025.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that the last fire drill conducted was more than six (6) months ago, on 05/16/2024, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they would conduct a fire drill and submit proof of completion to the SDRO by 04/18/2025.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that assistant did not have a current mandated report certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will have assistant take a mandated reporter course and submit proof of completion to the SDRO by 04/18/2025.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in that both licensee and assistant did not have a current Pediatric CPR and First Aid card on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they and their assistant will take an EMSA approved Pediatric CPR and First Aid course, and submit proof of completion to the SDRO by 04/18/2025.
