LEOS VALDIVIA, MARGARITA FAMILY CHILD CARE

Based on record review, the licensee did not comply with the section cited above in one out of one infant under 12 months child #1 (C1) not having an Individual Infant Sleeping Plan (LIC-9227) as required, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Margarita Leos Valdivia stated that she will ensure to fill an Individual Infant Sleeping Plan for child #1 together with infant's parent. Licensee will ensure to send a copy of the completed LIC-9227 form to the San Diego Child Care Regional Office (SDCCRO) by the due date as proof of correction.

Based on observation, the licensee did not comply with the section cited above in 13 out of 13 children present were not school age and facility was over capacity by one child which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Provider Margarita Leos told LPA that she will ensure to dismissed and terminate one of the children to ensure to be in compliance. Provider will send a written statement to the San Diego Child Care Regional Office (SDCCRO) advising that she read and understood section cited.

Based on interview and record review, the licensee did not comply with the section cited above by not renewing the required Pediatric 1st Aid/CPR that has been expired since 02/2023 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Provider Margarita Leos stated that she will ensure to take and complete the required 1st Aid/CPR certification and will send a copy to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.
