Resolution: Licensee submitted a written statement that states she understands the regulation above. Licensee states she will not use the converted garage as by 6/23/23. Licensee states she will not be using the converted garage until it is cleared and licensed by the department.
Based on observation, interview and record review, the licensee did not comply with the section cited above by not notifying the department of using the converted garage for daycare space, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will send the department the city permit documentation for use of garage to be livable space, the application LIC279, a statement of changes, and an updated facility sketch by 11/3/21.
