Based on observation, the licensee did not comply with the section cited above by having a bouncer in the daycare room and licensee stated she uses it for infant #1, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Provider immediately removed bouncer out of the reach of children during inspection. No follow up plan of correction needed.

Based on record review, the licensee did not comply with the section cited above in one out of one infant present in the facility, did not have sleeping log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Provider stated that she will start documenting the infant's sleep and send a copy of the sleeping log to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.

Based on analyst record review, the licensee did not comply with the section cited above for the three infant children present today as she has not been keeping safe sleep logs for infants which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee was given a sample of a safe sleep log and will begin documenting all infants' napping activity as required beginning 04/25/23. Licensee stated she will send analyst a copy of the logs for any two of the infants in care for 04/25/23 to 04/28/23 by 05/01/23 for him to review to complete the correction. Licensee also understands she will always continue to log all infants napping time as required.

Resolution: Licensee stated that she and her assistant shall complete a new training class at the Mandated Reporter Training website at mandatedreporterca.com and submit copies of their completed, updated certications to analyst by 05/29/23 to complete the correction.

Resolution: Licensee was provided a blank copy of an emergency drill log and states she will conduct a fire and disaster drill with the day care children and enter into and post the log and then submit a copy of the completed drills to analyst by 05/01/23 to complete the correction. Licensee understands she will continue to conduct both drills once every six months.

Resolution: Licensee states she will transfer all the children's shot records to the PM 286 vaccination cards as required and send analyst a copy of the cards of any two children of her choice by 05/01/23 to complete the correction.

Resolution: Licensee was provided with a blank copy of the form today and stated that she will have the parent complete the form and keep the copy of it in his file and then submit a copy of it to analyst by 05/01/23 to complete the correction.

Resolution: Licensee was provided with a listing of the shots that are required to be documented for all care providers and stated would obtain copies of those records from her doctoror or obtain the required shots or confirmation tests demonstrating immunity and send a copy of those to analyst by 05/29/23 to complete the correction.

Based on analyst observation, the licensee did not comply with the section cited above as two of the three playpens in the home had mattresses that were smaller than the size of the playpen in which the were installed which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states she will purchase the appropriate sized mattresses for the playpens and send analyst photos of them now in the respective playpens, as well as copies of purchase receipts for them, by 05/01/23 to comlpete the correction.
