(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).
Based on LPA’s observations and interviews conducted, the licensee did not ensure the personal rights of children in care with safe and healthful accommodations as 18 children on 1/12/2022 and seven (7) children on 1/24/2022 were not wearing face coverings while indoors as required by the State Public Health Officer Order dated June 11, 2021, which poses an immediate risk to the health, safety or personal rights of children in care.
