Based on observation and Licensee's statement that she was unable to provide a copy of the Mandated Reporter Training Certificate, the licensee did not comply with the section cited above in that she was unable to provide copy of the Mandated Reporter Training course, which poses as a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she will complete this online course and provide LPA with a copy of her training completion certificate no later than 05/20/2024.
