Based on observation and interview, the licensee did not comply with the section cited above in that one (1) out of two (2) infants were seen sleeping in a pack and play with a blanket, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Staff #1 (S1) stated that they would remove the blanket and will ensure cribs stay clear of loose articles and objects. S1 also stated that they will review Safe Sleep regulations provided by LPA and submit a brief summary to the San Diego Regional Office (SDRO) by 11/28/2025.

Based on observation and interview, the licensee did not comply with the section cited above in that they were away from the home for more than 20 percent of the hours that the facility is providing care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's daughter will give the provided the printed regulation to the licensee for review. Licensee will write a statement acknowledging receipt and acknowledgement of the regulation and provide a copy to the SDRO by 11/28/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that one (1) of two (2) staff do not have a personnel file and do not have copies of immunizations available for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create a personnel file for Staff #2 (S2) and include a copy of their immunization record on file. Licensee will provide proof of correction to the SDRO by 11/28/2025.

Resolution: Licensee will immediately cease use of the converted garage area and submit an update LIC999A to the SDRO and get an updated fire clearance by 11/28/2025. Licensee will not use the converted garage until approved by the SDRO.
