MENDEZ DAY CARE

Based on obsercation of the built-in pool, licensee did not have a key-lockable device on the pool gate, pool alarm in the pool, 12 foot rescue pole with body hook, life ring, or pool log. The pool does not meet AB2866 regulations. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase and install a key-lockable device on the pool gate, a pool alarm, a 12 foot rescue pole with body hook, a life ring at least 17 inches in diamter, and begin a pool daily inspection log to ensure compliance with AB2866 regulations. LPA will conduct an unannounced inspection after 08/29/2025 to ensure compliance.

Based on record review, licensee did not have current, completed and documented 15-minute sleep logs for child #1, child #2, and child #5. The last documented sleep logs were completed on 08/01/2025. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will check on sleeping infants and document on safe sleep logs for all infants in care. Licensee will submit 2.5 weeks worth of safe sleep logs to LPA Pacheco by POC due date 08/29/2025.

Based on record review the licensee did not comply with the section cited above in 2 out of 5 children's records files. Form LIC 9150 is missing from children's file which poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee stated she will obtain form LIC 9150 from parent's for children's files by POC due date 02/24/2023.

Based on observation of staff files, LPAs did not observe a current AB 1207 certificate for Staff #1. This poses a potential health, safety or personal rights risk to children in care.

Resolution: Staff #1 stated they would complete the required training and provide documentation to Community Care Licensing by 5/25/22.

Based on observation of staff files, LPAs did not observe a current documentation of immunization against the measles for Staff #1. This poses a potential health, safety or personal rights risk to children in care.

Resolution: Staff #1 stated they would obtain documentation that they are immunized against the Measles and provide documentation to Community Care Licensing by 5/25/22.
