Resolution: Licensee stated Mandated Reporter Training will be completed and submitted by 10/18/2024.
Based on observation, the licensee did not comply with the section cited above. LPA observed debris such as a broken pipe, broken beverage refrigerator and spoiled fruit on the ground in the backyard. This poses a potential health, safety or personal rights risk to children in care.
Resolution: Licensee stated she will have the items removed from the accessible area of the backyard and provide documentation to Community Care Licensing by 7/13/22.
Based on observation of personnel files. LPA did not observe immunization records for Staff #1 that they have been immunized against influenza, measles and pertussis. This poses a potential health, safety or personal rights risk to children in care.
Resolution: Licensee stated she will have Staff #1 obtain their immunzation for influenza, measles and pertussis and provide documentation to Community Care Licensing by 7/13/22.
Based on observation of facility records. LPA verified with Fresno Regional Office that Staff #1 is fingerprint cleared but not associated to this facility. This poses a potential health, safety or personal rights risk to children in care.
Resolution: Licensee stated she will have Staff #1 submit fingerprint clearance transfer paperwork to the Fresno Regional Office by 7/13/22.
