Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the staff file review the LPA was unable to review a Mandated Reporter certificate for S2.

Resolution: Licensee all staff working with the children must have a current Mandated Reporter certificate on file. Training must be retaken every two years. Licensee agrees to have S2 complete training and will submit certificate of completion to the Department on or by POC due date of 7-24-2025. Training can be completed at www. Mandatedreporterca .com

Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review the LPA was unable to review the PM 286 for C1, C2, C3, C4 and C5.

Resolution: Licensee understands she must transfer all children’s immunizations to the PM 286 and keep on file for review during inspection. Licensee agrees to transfer immunization records to the PM 286 for C1, C2, C3, C4 and C5 and submit to the Department on or by POC due date of 7-24-2025.

Based on record review, the licensee did not comply with the section cited above, an emergency drill had not been conducted since April 2023, which is more than 6 months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct an emergency drill by POC date and will send an updated log to LPA.
