HUANG & CHEN FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in that there were 5 items that contained the label "Contact Poison Control" if ingested. These items were located in the children's restroom and inaccessible to children behind 2 child proof latches. However, the storage area for them was not secured with a key lock or combination lock, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit proof (pictures) of the 5 items considered poisons in a storage area secured by a key lock or combination lock to the Department by the POC due date.

Based on record review, the licensee did not comply with the section cited above in that 3 out of 5 children's records (Child 2 [C2], Child 3 [C3], and Child 4 [C4]) did not contain a completed LIC995A form, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit proof (copy) of a completed and signed LIC995A for C2, C3, and C4 to the Department by the POC due date.

Based on record review, the licensee did not comply with the section cited above in that 1 out of 1 children's records (Child 2 [C2]) did not have a completed LIC9227 Individual nfant Safe Sleeping Plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit proof (copy) of a completed LIC9227 for C2 to the Department by the POC due date.
