GROSS FAMILY CHILD CARE

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the facility records review the LPA reviewed a fire drill that was last conducted on 12-4-2024.

Resolution: Licensees understands fire drills must be conducted at least once every six months. Licensees agree to conduct a fire drill, document and submit to the Department on or by POC due date of 7-4-2025.

Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the review of children’s file the LPA was unable to review immunizations on the PUB 286.

Resolution: Licensees understands all immunizations received by the Authorized Representative(s) must be transferred to the PUB 386 for all children enrolled. Licensees agree to transfer immunizations for all children enrolled and send the PUB 286 and send to the Department on or by POC due date of July 4, 2025.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 infants in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees agree to have the LIC9227 completed and sent to the Department by POC date. Licensees agree to have all infants under the age of 12 months complete the form going forward.

Based on record review, the licensee did not comply with the section cited above in 3 out of 3 infants in care which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have an ongoing sleep log for all infants in care until the infant reaches the age of 24 months. Licensee will send the logs to LPA by POC date.

Based on observation, the licensee did not comply with the section cited above in 1 out of 2 pack n plays there was a blanket present with the infant in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees agree to review the regulation and write and sign the letter of understanding, this can result in an type A violation with a future visit if this regulation is not in compliance.Letter will be sent to LPA by POC date

Based on Infant Record Review, the licensee did not comply with the section cited above in 2 out of 2 Infant Files were missing the Infant Sleeping Plan LIC 9227 which poses a potential health risk to children in care.

Resolution: Licensee stated she will make copies of the LIC 9227 provided and provide parent's signed copies to department and place in Children's file by due date.

Based on Record Review, the licensee did not comply with the section cited above, 2 out of the 2 Infant Files reviewed did not have a Documentation of an Infant Sleeping Log available for review which poses a potential safety risk to the children in care.

Resolution: Licensee stated she will create an Infant Sleeping Log for both Infant's in care and add to the Infant's File, completed Sleeping Logs will be provided to department by due date.
