Based on observation, the licensee did complete the 15 minute safe sleep checks, however the licensee did not comply with the section cited above in that the licensee did not document the 15 minute safe sleep checks for Child 4 (C4) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: License will provide proof (pictures) of 1 filled out 15 minute safe sleep log for C4 to the Department by the POC due date.
Based on interview of licensee and record review, the licensee did not comply with the section cited above in [2] out of [2] [L1 and L2) (Licensee and assistant], which posess a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Assistant agrees to complete Mandated Reporting Certificate and submit proof of completion to LPA Steven Montoya via email. (steven.montoya@dss.ca.gov) by 8-24-2023. .
Based on record review, the licensee did not comply with the section cited above in [1] out of [2], (L1 or L2) (Licensee or Assistan did not have current First Aid CPR., which posess a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete and submit current First Aid CPR and email to LPA Steven Montoya via email by 8-31.2023. (Steven.montoya@dss.ca.gov).
