Based on record review, the licensee did not comply with the section cited above in that there were no personnel records available to review for Staff 2 (S2) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit proof (copies) of all the listed items: (1) Employee's full name. (2) Date of employment. (3) Current home address and phone number. (4) A signed and dated copy of the Notice of Employee Rights [LIC 9052, (Rev. 03/03)] as required by Section 102416(a) and Section 102417. All of the above items will be submitted to the Department by the POC due date.

Based on record review the licensee did not comply with the section cited above in that there were no immunizations documented for Child 3 (C3) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit proof (pictures) of immunizations for C3 to the Department by the POC due date.

Based on record review, the licensee did not comply with the section cited above in 5 out of 5 infant safe sleep logs, 2 infants were present today, which poses a potential health, safety risk to persons in care.

Resolution: Licensee agrees to complete a sleep log for every infant in care, Licensee will send a copy of the sleep log to LPA by POC date.
