Based on observation, the licensee did not comply with the section cited above in that 1 infant, Child 1 (C1): was sleeping in a bassinet, with a mattress that did not fit, with a sheet that did not fit tightly onto the mattress. Each of which does not comply with Title 22 regulations, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a written statement (signed and dated) acknowledging their understanding of CCR sections; 102352(p)(2), 102425(a), 102425(a)(3), and 102425(a)(4). Also, licensee will obtain and send proof of (pictures) of an appropriate crib or play yard that will be used instead of the bassinet. Both of these items will be sent to the Department by the POC due date.

Based on observation, the licensee did not comply with the section cited above in that LPAs observed 1 infant, C1, in care that was inside of a sleep sack while sleeping, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee removed the sleep sack from the infant immediately upon notification of this regulation, brining the facility back into compliance with this regulation during this visit.

Based on record review and interview, the licensee did not comply with the section cited above in that LPAs reviewed records which showed that Child 3 (C3) did not have 15 minute safe sleep logs available to review for the past year, which poses/posed a potential health, safety or personal rights risk to persons in care. The other infant in care, Child 1 (C1) did have their 15 minute safe sleep logs completed. Additionally C3 will age out of this requirement within 1 month.

Resolution: Licensee will submit proof (copy) of 2 different days of completed safe sleep logs for C3 to the department by the POC due date.
