Based on observation and record review, the licensee's did not comply with the section cited above in 2 out of 2 licensee's did not complete the mandated reporter training required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's agree to complete the required training and send a copy of the certifcates via email to LPA Bianca Mnedez at binaca.mendez@dss.ca.gov by COB 10/25/2024.

Based on observation and record review, the licensee did not comply with the section cited above in 2 out of 2 infant children requiring 15 minute check which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to start conducting 15 minute infant sleep checks for C1 and C3 as required. Licensee's will provide a copy of the sleep logs via email to LPA Bianca Mendez at bianca.mendez@dss.ca.gov by COB on 10/25/2024.

Based on observation, the licensee did not comply with the section cited above in 1 count of infant sleeping with blanket in pack and play which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Liceenses will watch safe sleep video on CCL website provided to Licensees by LPA Sims and send statement verifying both licensees watched the video by 11/22/2023 to LPA Sims via text (916)365-5713. Licensees will have all parents of currently enrolled children sign LIC 9224 by next buinsess day or next day child is in care. Licensees will also have all parents who enroll children for the next 12 months sign LIC 9224. LIC 9224 shall be maintained in children's files.

Based on observation during child file reviews, C6 was missing all enrollment paperwork.

Resolution: Licensee agrees to obtain all enrollment paperwork and provide proof of correction to CCL by 11/28/22.
