Based on record review, the licensee did not comply with the section cited above in S1 missing LIC 503, health screening which poses a potential health, safety or personal rights risk to persons in care.

Based on the LPA's interview with the center director on 04/28/23, the center director stated that the facility did not obtain signed LIC9224 “Acknowledgement of Receipt of Licensing Reports” from parents and guardians of each child receiving services in this facility. The facility was cited a Type A violation on 01/04/23. This poses a potential health and safety risk to children in care.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidence by: Staff left two children in care on the playground for approximately 1 minute which poses a potential health and safety risk to children in care.

Based on interviews with facility staff on 04/28/23, it was corroborated that due to a lack of supervision, child C1 cut their hair with a pair of children's scissors found in the classroom; this was not observed by any staff member. This poses a potential health and safety risk to children in care.
