Based on interview and record review, the licensee did not comply with the section cited above as LPA observed the last fire/disaster drill was conducted on 10/04/2024 according to Licensee's disaster drill log and Licensee interview which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document a new fire/disaster drill. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 07/08/2025.

Based on interview and record review, the licensee did not comply with the section cited above as the last fire/disaster drill is documented for 2020 and Licensee stated they have not done a new fire/disaster drill since then as they believed they were no longer required which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a new fire/disaster drill and agrees to document the drill on their log. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/21/2024.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee did not have an up to date Mandated Reporter Child Care Providers (AB1207) certificate on file nor proof of completion which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of a current non-expired Mandated Reporter certificate and agrees to maintain proof at facility. Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C3 was missing proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C3's immunizations and agrees to maintain proof at facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/21/2024.

Resolution: Licensee agrees to conduct and document the 15 minute sleep checks for C3. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 02/21/2024.
